What is needed from you:

Certificate of Insurance, including any listed exclusions, listing USA Truck, LLC. as the certificate holder and listing the following coverages:

• General Liability of at least $1,000,000.
• Auto Liability of at least $1,000,000.
• Cargo Coverage of at least $100,000.
• Trailer Interchange of at least $25,000.
• DOT safety score of “satisfactory” or “none.”
